在分析国内医联体双向转诊现状的基础上,结合宝安人民医院(集团)双向转诊信息化存在问题,通过建立双向转诊信息平台,充分发挥信息化建设对双向转诊的支撑作用,建立了畅通的上转、下转信息高速公路,实现了异构系统的互联互通及数据有效共享,推动了双向转诊有效落地。自2019年9月份双向转诊信息平台上线以来,医院(集团)下转社康中心人次明显增多,"转上容易转下难"的"单向转诊"局面得到明显改善。以高血压、糖尿病为例,院本部2019年9月份下转613人次,同比增长59.715%;院本部2019年9月份上转320人次,同比增长19.555%;双向转诊信息平台强化了医院社康融合发展,更好地落实了医联体内各医疗机构功能的定位归位,有利于全面推进分诊诊疗体系建设。

This paper analyzed the current situation of two-way referral of medical association in China, combined with the problems of two-way referral information in the People's Hospital(group) of Baoan, Shenzhen, to completely exert effort of information technology by establishing thetwo-way referral information platform.The platform establishes a smooth up turn and down turn information superhighway, which achieves interoperability between different information systems, and achieves the business strategy for its landing. The number of people transferred fromhospital(group) to social health center increased significantly, the "one-way referral" situation of "easy to transfer up and difficult to transfer down" has been significantly improved since the two-way referral information platform operation in September, 2019. Taking hypertension and diabetes as examples, the downward transfer was 613 times in September, 2019, it has soared by nearly 59.715% compared with last year; the upward transfer was 320 times in September, 2019, it has soared by nearly 19.555% compared with last year. The two-way referral system of medical association strengthenes the integrated development of between hospital and social health, better implements the positioning and homing of the functions of medical institutions in medical association, and is conducive to comprehensively promot the construction of triage diagnosis and treatment system.
